Student Sign-Up Form

I do hereby make application for student membership in Maka’s Taekwondo, LLC.  I wish to take part in Taekwondo.

If accepted for the indicated program(s),  I hereby agree to pay an instructional and/or exercise fee dependent of the type of membership I choose.  These fees shall be paid in advance and are non-refundable.  These fees will entitle me to supervised sessions of Taekwondo each week. If I wish to change my membership, I must give at least a 30-day notice before my membership expires. I understand that signing this document is commitment to a few things. First, coming to class regularly to see growth in mind, body, and spirit. Secondly, becoming a part of a determined and respectful team. Lastly, to fulfill my financial commitment as a betterment to the attendee. 

I hereby agree to abide by and obey all rules and regulations of Martial Arts and to conduct myself in and safe and proper manner at all times.  I further agree to hold harmless and full indemnify Maka’s Taekwondo, LLC, and any and all persons employed by or a member of the school for any and all injuries or damages that I might sustain or incur while present in the school or attending any functions in which the school or its representatives are involved.

I hereby state that I am in good health and have medical approval to engage in these physical training programs.  I further state that I hereby assume any and all risks that are a part of or incidental to these training programs.

I understand that this is a legally binding contractual Agreement, and that I have read this agreement in its entirety prior to my signing this Agreement.  I further understand that by signing this Agreement, I shall be legally bound by its terms and provisions.

RELEASE:  I, the undersigned, in consideration of being permitted to participate in course(s) and program(s) of instruction, training, contest and/or exhibitions involving the Arts of Taekwondo and Hapkido, do hereby release, remise and forever discharge the School, the instructor-in-charge and/or his assistants, other participants in instruction, training, contests, or exhibitions, and all of the aforementioned parties, their heirs, administrators and assign, from all manner of actions, causes of action, demands that may result from or be incidental to my participation therein.  This release if given for the purpose of inducing the School to allow me to enter into and participate in the above mentioned Taekwondo activities.
